Has anyone had to fill out a patriot act for an LLC ?
I’m wondering what to put for the forms of identification and date of birth

I would presume you put in the name and info of the authorized rep/manager/member who is signing on behalf of the LLC.

In my experience, an “LLC” does not sign a loan application, but a owner/manager (depending on articles of organization) does. That’s who’s name goes on the form. Usually someone who is authorized to conduct transactions on their banking accounts as well. Bottom line, a name of a person, not a company. Hope this helps!
