Does a address affidavit need a certificate attached?
If the doc does not have notary language at the bottom, look for key words in the doc -
“The undersigned, being duly sworn, deposes and says…”
"The undersigned hereby swear that … "
“Affidavit” indicates a jurat - BUT if the language isn’t already there you cannot make that determination - you need to call hiring party and ask them what they want

Ethically that’s who you contact unless they have given you instructions to deal with title directly; otherwise everything goes through them.
