Help with PCOR & the Fire Insurance forms

Hi everyone Newbie here, who completes the PCOR & the Fire Insurance Authorization forms? Other than the Patriot Act are there any additional forms SA’s complete? Thanks for the help in advance

According to the blurb at the top of the PCOR it’s to be completed by the buyer.

To be completed by the transferee (buyer) prior to a transfer of subject property, in accordance with section 480.3 of the Revenue and Taxation Code. A Preliminary Change of Ownership Report must be filed with each conveyance in the County Recorder’s office for the county where the property is located…”

Not sure what the Fire Insurance Authorization Form is but are there instructions on the form?

Technically, we’re not supposed to be completing any forms for them except those we are involved in - meaning like the Patriot Act form where WE are saying they gave US the following items as identification.
