Page 1 what do I put in these lines?

OR, do I not fill this out because it is for the county to notarize when it is filed?

The venue should be the county in which the notarial act took place. On the first part if you have a question on whether or not something needs to be filled in by the signer or you, call Title and ask them. It is better to ask if you’re unsure than to assume and either do something that shouldn’t be done or not do something that should be done.
Cheryi you need to get some training before you get in trouble.
You are doing the right thing by asking questions to your fellow colleagues. I do agree with @earl226, that you need to seek additional training on notarial certificates or seek out mentorship with a seasoned LSA in person or virtual and the mentor doesn’t have to be located in your state. Be careful when asking title how to complete notarial blocks, that’s our responsibility to do so, not the title company. Keep in mind some of these title companies are nationwide and their staffs are not up to speed on each state’s notary satutures requirements.

Thank you for your response and I do have someone to help me, however, they were not awake…documents weren’t sent till late last night and closing was at 8am, 45 minutes away. That is why I sought out a little help here…
Agree, I am not afraid to ask. Better to ask and get the " you need to learn" and the helpful “yes, do this with instructions to help a fellow out” then to not ask and issues after the fact come about.
