This notarization form looks like it would be for a RON signing. The line by the checkbox would be for the notary to check to indicate this was being done by remote electronic notarization. The location could be used for your venue. You could put N/A in the date section to clarify that you are a standard notary. However, the form looks incomplete. It is missing a place for your signature as well as your commission number and date of expiration.
In general, I just leave things that don’t pertain to me alone. If title decides it’s an error, the duty to fix it is on you for doing it.
