Hello! Has anyone had experience in notarizing a passport? What is needed?

This might help, here is what i use Online when I get that request.
AFFIDAVIT OF TRUE COPY (Passport)

(1) I am the lawful holder of the passport described below
(2) The attached document is a true, exact, and complete copy of my original Passport.

Make sure this is acceptable in your state before using it.
Linda, this is the reason I will not provide comments or assistance. The certificate clearly states that you are not certifying the passport as a true copy. Rather, you are administering a sworn (or affirmed) certificate in which the client attests that they made the copy and that they are the custodian of the document. Failure to recognize this distinction reflects a lack of review of the certificate.
@antonio I was addressing this to @loricnotary22 - not you. I did read the cert and it’s clearly an Affidavit of the Document Custodian. This may not be acceptable in OP’s state
