Blank document for Annuities signing

I have received a package for an annuity. It is all blank except for the owners signature portion. which has the owners name to be signed. Is that correct? Would you notarize those documents?

Here in Florida we are not allowed to notarize any document if it’s blank or incomplete. I would check with hiring party. Sounds like someone goofed.

(CA) I’d never notarize a blank document. I understand we are authenticating someone’s identity and witnessing them sign documents, but, in blank?? Who knows what could be superimposed in that blank space that will surely get the notary in trouble later. Not me, brother. I’d pass.

The ones I’ve seen mostly require a lot of completion of blanks by signer.
Tho’ the page count is fairly low, the table time is quite high.
