Does the spouse sign this form as well? Only the husbands name is printed on it.
Normally the spouse has their own to sign…I’d check with hiring party

she might be a non-borrowing spouse
typically the only persons signing are the names listed on lines 1A and 2A. If spouse is not listed in 2A, I do not have them sign
