Should the Borrower’s name go in this section or do I leave it blank? and is there anything else I need to be aware or bring to the Borrower’s attention concerning this form? Reading the instructions in 37A & 38A: initial if veteran and/or spouse do not wish to provide info for 37B, 37C, 37D, 38B, 38C & 38D then date and sign. If 37B, 37C, 37D, 38B, 38C & 38D are already checked, no initials, just date and sign.
To answer OP’s question, no. No need to enter veteran’s name in 37A nor the co-obligator’s name in 38A.
If the check boxes aren’t rechecked, either ask the client to check the demographics portion or initial if they prefer not to answer. It has worked for me.
