Does anyone know who is supposed to sign this extra DOT page? It’s one page labelled DOT on top of the full DOT? Thank you.
Actually, you should call the Title company and ask them what they want the borrower to do on that random form. This is not for YOU to figure out. Make Title do their jobs. If you are at the table with your client and you still haven’t gotten a reply from Title, have your Client call their Loan Officer.

Agreed with @Patricia is the TC or escrow that needs to fill the form with signer’s name.

I just had one of these forms witj a notorial cert attached. Cpuld not get hold of title company. I looked at it closer and the signers name was not on this one. I left it blank and sent it back with the package and never heard back fro the title company.

It’s for reconveyance when there is a refi or payoff, not the borrowers to sign. for more explination go here
