Qiicktime Notary - "BARD" Signings

I got an order from Quicktime Notary to get some papers signed for the law firm of Bachus & Shanker to compete a BARD signing. After countering the fee I was assigned the case to be done ASAP.

The phone number I was given was not in service, so I made that note in Snapdocs, left a message at the law firm and sent an email to Quicktime Notary after calling and only got a ringing phone with no answer.

I read through the instructions below and realized they wanted you to drive there without confirming the appointment and if they were not there take a photo of the address and try to get information from others about where the signer may live.

This was my last comment on Snapdocs “As I mentioned in my earlier comment the phone number for Mr. Parks is not in service. I have called and left a message at your office. I am unable to proceed without having a working number and confirm that Mr. Parks will be there to complete and sign his documents.”

I have since declined this signing. I get documents signed. I don’t waste time going somewhere that doesn’t have a phone number is service and I’m certainly not a skip tracer or private investigator.

These are the instructions that came with the signing.

Welcome to the platform and thank you for helping us with this very important task. If you have any questions or need clarity on the order, call Storm DuBois at Quicktime Notary and he will be able to assist you with the order. Most of the questions should be answered below.

Introduction: Our clients are involved in a nationwide Bard Power Port Case. The courts are requesting documents to be signed. It is your job to reach the client and set an appointment, meet with client, and get these documents signed, and scan back to us using the Quicktime Scan Docs platform. Documents must be scanned back the same day as you receive the signature. You will then be provided with a shipping label when the order is complete.

APPOINTMENT – CALL SCRIPT – INSTRUCTIONS

Prior to calling client, carefully read all instructions below and see attachments to see what you will be needing the client to sign.

You will print these documents out and take them with you. Please make additional copies of the authorizations in case the client makes a mistake in signing the documents. If a document is not completed correctly, you will be asked to go back at your own expense to gather the missing information.

SCRIPT: When speaking with the client
Hello, I am calling on behalf of Bachus and Schanker Law firm out of Denver Colorado regarding your Bard Power Port case. I am calling you today because the courts in your case are requesting documents to be signed. I am in your area, and would like to make an appointment to bring you the documents.

Most likely the client will say that they already signed every document we needed, and in that case, you may tell them that this is a document that has not been previously signed and you have been assigned the task to bring them the document and have them sign it.

OUTGOING/APPOINTMENT CALLING INSTRUCTIONS:

  • Notary is to call and set appointments.
  • Try to call at different times of the day to reach the client within the first few hours of receiving this task.

Make an appointment. Confirm the address and ask if it is a house or apartment if not previously indicated. Ask if there is a gate code.

  • Some of our clients work and you may reach them in the evening. When you speak with the client, confirm the address and ask if the residence is a house or apartment. Sometimes when it is an apartment, we may not have the unit # listed for you.
  • Only after you have made up to 3 attempts within a 4-hour period to reach them by phone and have been unsuccessful, you may then go to address and door knock.
  • If client is not home; take a photo of the residence with the street address in photo such as near door or mailbox with house in background and scan back to portal.
  • In some cases, we do not have a good contact number for the client and in this case, you will go directly to the client’s last known address and ask for the client.
  • We need to know if our client still lives at this address, ask anyone you see in the area if they know the client and if he/she still lives there. If the client still lives in the address, leave the documents at their doors or mailbox. Our agent will follow up. You may also be asked to retrieve the documents later for an additional trip charge.

-If the client is deceased, look for the next of kin, leave the documents to the NOK, with our phone number and tell the NOK to contact our office. Also, get the NOK’s complete name, active phone and email address.

  • Please make all notes in the scan docs portal. If you obtain new addresses or phone numbers, please let us know by writing notes in the portal so we can update our files.
  • If you have been successful and set the appointment; continue to follow the instructions in attached documents and descriptions below: Print attached documents and take with you to clients.

Scan back filled out and signed forms. Do not notarize or sign any of these documents. Counsel is the attorney on file, please do not sign in this area. Make sure all instructions are followed to avoid a second trip at your expense.

Below are the attached forms and instructions.

  1. Questionnaire (3 pages): Please read all instructions carefully. Print this out and take it with you to the client’s appointment. Let the client know there are fast approaching deadlines in their case and these documents must be filled out promptly.

  2. Medical Release Form (1- page HIPAA Authorization): Client must fill only the a) Patient Name, b) Patient DOB, c) Patient SSN; d) Signature on Patient or Legally Authorized representative; e) Patient or Representatives Printed name. (If the HIPAA was signed by a representative, answer the Relationship)

  • Note you may want to make a few copies in case a mistake is made when filling the pages out.
  1. Attestation Form (1 page) – Sign only where it asked for a signature.
    a) “Signature of the person requesting the PHI” - if he/she is signing for her/himself
    b) “If you have signed as a representative of the person requesting PHI, provide a description of your authority to act for that person” – if the signer is a representative of of a deceased person

  2. PHI Form (1 page) - Sign only where it asked for a signature.
    a) Signature of the client – if the person is signing for themselves.
    b) Relationship – if you are signing on behalf of the client.
    c) Witness – if applicable.

  3. Implant Card or Sticker – Ask client to present their Implant Card or Sticker and take a picture of it, must be in a clear photo and all details are readable – Scan back to the portal

Scan back all docs and photos to QuickTime Snap Docs portal.

Had a $50 offer for one of these today from same co. thru SnapDocs. 6 page est. Turned it down without any thought as location city is waaay out of my area. May have accepted if it was a block away. so thank you for providing all the undisclosed in offer stips involved.

I’ve taken trips for attorneys on the Johnson & Johnson lawsuit. Sort of like that but I always got paid when discovering someone was dead. Or leaving a note.
